Straight guy with a serious question.

I'm asking in general. How do you decide who is top and who is the bottom? Do the roles reverse or mainly stay the same. Does it become a problem? Such as two tops not wanting to bottom for the other. I know there is no simple answer. Just want a general idea. Thanks.
